Smokefree South Gloucestershire

Smokefree South Gloucestershire is a local support to stop smoking service based in a number of settings including GP Surgeries, Pharmacies and other community venues. We work with people over the age of 12 providing advice, behavioural support and medication to help people to stop smoking.

 

What do we do with your data?

For a referral to be made to the stop smoking service, you gave your consent to the person who referred you.

The information that has been shared about you includes: your full name, gender, date of birth, ethnicity, address, contact details (telephone number and/or email), and your Carbon Monoxide (CO) reading. If applicable, your pregnancy and/or breastfeeding status has also been shared.

We will use your personal information to provide you with a service. We will only share your personal information with our partners who have referred you, or with those that are involved in your treatment, but no one else without your permission. Anonymised data may be shared with other healthcare professionals and the Department of Health. The information we share is what has happened within your episode of care and how we have achieved the objectives set by the Department of Health.

Upon completion of the service, your personal information will be kept for twenty years for those under the age of 18, and seven years for those over the age of 18. Statistical information about people who use the service may be retained for longer than this, but this information would not identify you in anyway.
